What hurdle did the Phase III success clear?

  • Intismeran, a personalized cancer vaccine developed by Moderna and Merck, achieved a Phase III recurrence-free survival hazard ratio (HR) of 0.58 in combination with Keytruda as adjuvant treatment after melanoma surgery. The result was statistically significant and cleared the threshold of below 0.60 that Goldman Sachs had identified in discussions with investors; the Phase II HR was 0.52, indicating that efficacy was largely retained in the larger trial.
  • At two years, 82.2% of patients in the combination group were alive and recurrence-free, versus 70.0% in the Keytruda monotherapy group; the proportions alive without distant metastasis were 88.5% and 80.2%, respectively. But the overall survival HR was 0.77, with a 95% confidence interval of 0.45–1.31, and had not reached statistical significance. Goldman Sachs therefore places greater weight on the established recurrence and metastasis benefits.

Why was efficacy stronger in the latter period, and what constraint does manufacturing pose?

  • The recurrence-free survival HR was 0.99 in the first six months and fell to 0.42 thereafter. Management explained that personalized manufacturing entails a median wait of about nine weeks for the first injection, followed by the activation and expansion of specific T cells. Goldman Sachs therefore believes that, as follow-up lengthens and the early waiting period accounts for a smaller share, the overall efficacy measure has room to improve.
  • About 17% of screened samples could not support manufacturing because of insufficient tumor tissue. Experts attributed this partly to the operational learning process early in multicenter recruitment and cited a case in which such screening failures ceased at one center after its procedures were updated; tissue collection and handling remain practical barriers to patients entering treatment.

Will preoperative treatment displace postoperative opportunities, and what about safety?

  • Management and experts estimate that only about 20% of melanoma patients are suitable for preoperative treatment. Such regimens generally require accessible or macroscopic lymph node lesions, while many patients who need primary skin tumors removed or have stage II disease still rely on postoperative treatment. Experts consider SWOG S1801, which uses Keytruda, a more appropriate preoperative benchmark than NADINA.
  • In Phase III, grade three or higher adverse events occurred in 23.6% of the combination group and 20.3% of the monotherapy group; fever and injection-site pain were also more common. The study found no new safety signals. Of eight deaths recorded in the report, five occurred before the first dose of intismeran, and the remaining three were judged unrelated to the drug.

Why do pancreatic cancer data strengthen confidence in kidney cancer?

  • The key to expanding the platform is whether tumors with fewer mutations can provide enough targets and induce an immune response. In the Phase I pancreatic cancer study, all 19 evaluable patients showed vaccine-induced expansion of specific T-cell clones. However, among 20 treated patients, nine had relapsed and another 11 had neither relapsed nor died at a median follow-up of about 17 months. Goldman Sachs views this as preliminary mechanistic evidence.
  • Kidney cancer has a low overall mutation burden but relatively frequent insertion and deletion mutations, which can produce new frameshift antigens. This is the link underpinning Goldman Sachs’ optimism about expansion. Phase II kidney cancer data are expected at the end of 2026 or in 2027, depending on event accrual. Experts also noted that the benefits of cancer vaccines are currently concentrated mainly in postoperative adjuvant treatment, while metastatic disease still requires technological improvements.

The clinical assessment is positive, so why is the rating still Neutral?

  • Goldman Sachs rates Moderna Neutral, with a 12-month price target of 137 dollars, 31.5% below the closing price of 200.10 dollars on 23 October 2026. Its discounted cash flow valuation uses a weighted average cost of capital of 11%, and it raised the perpetual growth rate from 4% to 5% because of the oncology platform opportunity. Even with more optimistic long-term assumptions, the valuation remains below the share price at the time.
