Key takeaway
Citi acknowledges the progress of intismeran combination therapy in melanoma, but believes the current valuation requires success on several fronts simultaneously, including efficacy across cancer types, pricing and commercial penetration.
What did the melanoma data change?
Following the full disclosure of INTerpath-001 data, Citi believes intismeran has a prospect of approval and is clinically meaningful in melanoma. The benefit established by the trial came from adding intismeran to Keytruda.
Expert responses at the European Society for Medical Oncology (ESMO) meeting were positive but cautious. Citi views the treatment as an important addition to melanoma care rather than an obvious change in the treatment paradigm; discussions continued to address the maturity of overall survival data, patient selection, cost and competition from neoadjuvant immunotherapy.
Recurrence-free survival and distant metastasis-free survival were assessed by investigators rather than by blinded independent central review. Citi recognizes the clinical significance of preventing recurrence and believes the consistent distant metastasis data provide support, but mature overall survival data remain important for assessing long-term cost-effectiveness.
Where are the commercialization hurdles for personalized treatment?
The eligible patient pool is more restricted than previously understood: among 1,749 people screened, 17.3% had insufficient tumor samples and another 7.1% did not reach a disease-free state; ultimately, 1,137 people were randomized.
Among patients assigned to receive intismeran, product manufacturing was successfully completed for more than 99%, but this proportion excludes those screened out because of insufficient tissue samples. Commercial use depends on the entire process from sampling through manufacturing and administration.
The median time to first dose was approximately 64 days, slightly longer than the stated 6- to 8-week process. Citi believes replicating that speed at less experienced centers may also be challenging.
How much platform value can melanoma efficacy support?
Citi believes success in highly immunogenic melanoma is not yet sufficient to support the broad platform value implied by the share price. The bank believes the valuation already incorporates several outcomes, including efficacy across cancer types, favorable pricing and substantial penetration.
The idea of replacing Keytruda with monotherapy remains theoretical: the existing randomized trial established the incremental benefit of combination treatment. In a phase-one pancreatic cancer study, 19 evaluable patients developed neoantigen-specific T-cell responses, demonstrating manufacturing feasibility and immune activation, but Citi believes the data have not yet shown antitumor efficacy.
Kidney cancer is the next major test, with interim phase-two data expected by the end of 2026. Citi notes that kidney cancer has lower immunogenicity while expectations are already high; data that only support continued development could disappoint investors.
How does Citi value Moderna?
Citi maintains its sell, high-risk rating and a price target of 80 dollars, implying approximately 60% downside from the 200.10-dollar closing price on 23 October 2026. The target is based on discounted cash flow, using a weighted average cost of capital of 9.1% and a terminal growth rate of 2%.
If intismeran achieves better-than-expected clinical success in more cancer types, or personalized manufacturing scales faster and more efficiently, the company’s revenue and profitability could exceed Citi’s expectations; conversely, limited efficacy across cancer types or constraints on manufacturing and access could slow commercial rollout.
